Keeping you in the picture about your rights to privacy.

By law and together with my own personal regard for you and your privacy I am required to be transparent abut the nature of the data I collect from you and how this is stored, used and eventually destroyed.

Your personal data will be:
• Adequate, relevant and limited to what is necessary
• Accurate and, where necessary, kept up to date

This is a summary of the details I will keep:
Your contact details (e.g.: name. address, DOB, GP details)
Therapy contract (day and time we meet, your goals for therapy)
Assessment (including an outline of your current presenting problems and past history)
Session notes (a brief outline of what was discussed, key themes, areas worked on / to work on)
All paper records are stored and filed in a lockable filing cabinet
Text and email communication will be limited to discussions and notifications around sessions timings / cancellations / changes only and will not include discussion around personal information. This is to minimise any opportunity for internet data theft. An exception to this may be if concerns around risk and vulnerability are raised and where guidance around a crisis intervention might be required.
Electronic records will be kept to recording of session dates and payment details only and will not include your full name.
Invoice details (if required) will not include any personal information other than your name, session date, payment details and both your organisation’s and my address.
All records will be kept for 7 years after the end of our therapeutic work and destroyed appropriately (paper based shredded and electronic safely deleted)

I use the zoom.us platform, which is highlighted by my governing bodies as a safe and suitable platform. Although zoom states this is an encrypted platform, I am unable to guarantee your privacy on line. I do however take steps to ensure your session is private and confidential. I do not record these sessions and would ask if you would like to record that you discuss and agree that with me in advance.

I will not share any of your information without your prior knowledge and always preferably with your consent. The only exceptions to this are where there is serious risk of harm to you and or where I am required by law to release any information as outlined in our contract.
I have full professional indemnity insurance http://www.howdengroup.co.uk and I am registered with the Information Commissioners Office (ICO) ‘an independent authority set up to uphold information rights in the public interest, promoting openness by public bodies and data privacy for individuals.
